Your first step to becoming a volunteer SES member and joining the team at Chelsea SES begins here.

Firstly, fill in the on-line application form, CLICK HERE FOR THE APPLICATION FORM. Once completed, ensure that you print 2 copies of the form as the completed form cannot be saved, giving you a copy for us and a copy for your records.

Secondly, CLICK HERE to download the Police Check application form.

If you are having difficulty in viewing or downloading this form, please RIGHT CLICK on the link above and choose 'Save As'.

Once again, when completed, ensure that you print 2 copies of the form as the completed form cannot be saved, giving you a copy for us and a copy for your records. Like every SES volunteer, you will need to complete this prior to acceptance to the SES. Within this form you will also find the identification requirements (100 Points) for this to be done and the documents that you can use to  make up the points required. You will need to bring the completed forms and your 100 points of identification (as per Police check) with you when you come for your interview at our LHQ.

IMPORTANT NOTICE: The police check must be signed by the appropriate authority (listed in the form) before your application can be submitted

Some important things to remember when getting your Police check signed:

  1. When getting Police check form signed by appropriate authority (Police), you must ensure that the 100 points of identification and page 5 of the form, must be signed by the same person.
  2. Please take original documents of your 100 points of identification as well as photocopies for the Police to inspect before signing the copies.
  3. It is vital that the address on all documentation must match the address entered on the Police check form.

Once you have completed and printed both forms and your Police check has been signed by the appropriate authority and you have your 100 points of identification; call and leave your contact details on 9776 0721 (message only) and we will contact you to arrange an interview time at the unit.
